Important Forms

• Official Withdrawal from Program  (This form is for students who are not enrolled in any courses, and who are withdrawing from their program entirely.)
• Retroactive Add/Drop  (This form requires further processing within 15 days of receiving the approved form at their stonybrook.edu email address.) 
• Change of Program Form  (This form is for students who will no longer be associated with their cohort, but want to continue their program as a traditional online or on-campus student.)
